To qualify, the doctor being referred must place an order for a BruxZir® Zirconia crown or Comfort H/S™ Bite Splint within 30 days of referral submission. In addition, the doctor being referred will get their first unit of a BruxZir or Comfort H/S Bite Splint for FREE.
Upon submission of this form, an email with this information will be sent to the office providing the referral, the office being referred, and Crown World.

The Crown World Referral Program is limited to existing Crown World customers with a valid Crown World account number. The account must be current in their payment status to qualify. A limit of 5 referrals can be made per account and a maximum of 5 payouts will be made per account number. Please, no spamming. Offices submitting more than 10 referrals will be disqualified.
In the event a doctor is referred by multiple submissions, only the first submission qualifies.
All payouts will be sent to the official address listed for the Crown World account number. Payments will be addressed in the name of the person listed in the referral submission. Crown World is not responsible for any lost payments, and once sent Crown World will not replace any lost payments.
To qualify, the office being referred must open a new account with Crown World within 30 days of the submission and place a signed order for a BruxZir dental restoration or a Comfort H/S Bite Splint. The order must be received at Crown World offices within 45 days of the submission. Crown World offers free round-trip shipping. After opening an account, the referred office will receive a free welcome kit with supplies and pre-paid shipping labels.
Per this agreement, a new doctor is defined as an individual with a valid license issued by their state of residency to practice dentistry within that state. To qualify as new, the doctor may not have submitted any orders to Crown World Dental within the last 24 months, regardless of different billing account locations. New doctors added to an existing account at the same location do not qualify.
Crown World may terminate this promotion at any time and for any reason. The decision to pay a reward to the person making the referral is solely at Crown World’s discretion. However, in the event the promotion is terminated, any single BruxZir crown or Comfort H/S orders placed by the referred office prior to the termination will either be made at no charge or returned to the referred office at no charge.
